In order to qualify for the Early bird offer, all you need to do is pre-register before Friday 21 May and pay a single administration fee of only £35, thereafter £50. Full entries must be submitted by Friday 30 July.
For each category entered, please submit a separate case report consisting of between 500 and 1,000 words, as well as any supporting material as further evidence. Explain why your practice or the individual/team deserves to win. You may present as much material as you wish to support your claims such as photographs, patient testimonials, PR or marketing material (pictures on CDs and DVDs will be accepted but mus;#t be accompanied by hard copy printouts). The more effort you put into your entry the more impressed the judges will be. You can nominate yourself, a team or an individual and all entrants will be judged on their own merits. Judges or editorial staff will not enter into correspondence regarding competition entries.
